Preheat your oven to 375°F and lightly grease a 9x9 baking dish or pie dish.
In a large skillet over medium heat, cook the sausage until fully browned and cooked through. Drain any excess grease.
Reduce the heat to medium-low. Add the cream cheese and sour cream to the skillet with the sausage. Stir continuously until the cream cheese is melted and smooth.
Stir in the garlic powder, parsley, diced jalapeños, and shredded Monterey Jack cheese. Mix until everything is fully combined and the cheese is melted.
Transfer the sausage mixture into the prepared baking dish and spread evenly. Sprinkle the parmesan cheese over the top.
Bake for 15 minutes, or until hot and bubbly.
Remove from the oven and top with chopped green onions before serving.
Serve warm with tortilla chips.
